The goal is often to counteract the effects of gravity, pregnancy, weight fluctuations, or aging. While these procedures are highly effective, like any surgery, they have potential long-term changes. One of the most discussed is a phenomenon known as "bottoming out." As you can see in the photo above, this patient experienced bottoming out after a previous surgery elsewhere, and we were able to achieve a more balanced and natural result with revision surgery. Understanding this potential outcome is crucial for informed decision-making.

What is Bottoming Out Deformity?
Bottoming out deformity describes a specific change in breast appearance that can occur months or years after a breast lift or reduction. Essentially, over time, the internal breast tissue (glands and fat) naturally settles downwards due to gravity. However, the skin envelope and the surgically repositioned nipple-areola complex (NAC) tend to remain relatively fixed in their higher position.
This differential settling leads to:
Downward Migration of Breast Tissue: The bulk of the breast volume shifts towards the lower pole (the bottom part of the breast).
Relative Upward Pointing Nipple: As the tissue below it descends, the nipple appears to tilt upwards.
Upper Pole Emptiness: The upper part of the breast can lose fullness, contributing to a less rounded and sometimes "operated-on" look.
Increased Distance: The distance between the nipple and the inframammary fold (the crease beneath the breast) lengthens disproportionately.
The result is a breast shape where the nipple seems too high, the lower pole is overly full or sagging, and the upper pole looks deflated.

Why Does Bottoming Out Happen? A Natural Process
It's important to understand that bottoming out isn't necessarily a sign of surgical error; it's a potential consequence of the natural interaction between gravity, tissue weight, and skin elasticity over time. This complication can occur, to varying degrees, with virtually all standard breast lift and reduction techniques. The heavier the breast tissue and the weaker the skin quality, the higher the likelihood of some settling.
My Approach: Planning for the Long Term
Recognizing the tendency for tissues to settle, my approach during breast lift and reduction surgery involves anticipating this change:
Strategic Nipple Placement: I often position the nipple-areola complex slightly lower on the breast mound than where it might seem "perfect" immediately post-op.
Internal Tissue Over-Correction: I employ techniques to provide robust internal support and slightly "over-lift" the breast tissue internally.

The Healing Journey: Initial Appearance vs. Final Result
This proactive approach means that in the initial months following surgery (typically 1-3 months), the breast shape might look slightly "unnatural." The nipples may appear to point slightly downwards, and the upper pole might seem quite full. This is intentional.
As the swelling subsides and the breast tissues naturally settle with gravity over the subsequent months, this initial "over-correction" allows the breast to relax into a more natural final position. The nipple typically settles into a forward-pointing orientation, and the breast maintains better upper pole fullness compared to techniques that don't account for this settling process. The goal is to bypass the eventual bottomed-out appearance and achieve a result that looks natural and remains stable for longer.
