Case Description

This patient traveled from Italy to Istanbul seeking comprehensive facial rejuvenation. Her main concerns were sagging along the jawline and neck, loss of facial volume, and changes in skin texture that had developed over the years.

During the consultation, it was clear that addressing only one aspect of her aging would leave the result incomplete. Skin laxity was present in both the face and the neck, the deeper facial tissues had descended, and the skin surface had lost its smoothness and firmness. To achieve a truly natural and complete rejuvenation, a multi-layered approach was planned — working on the deep structural support, restoring lost volume, and improving the skin surface all in the same session.

Deep Plane Face and Neck Lift

A deep plane technique was chosen for this case. Unlike more superficial facelift methods, the deep plane approach releases and repositions the SMAS layer along with the overlying fat and skin as a single unit. This allows the tissues to be lifted vertically rather than simply pulled tight, which is key to achieving a natural, non-operated appearance.

Because the patient had significant laxity in both the face and the neck, performing both procedures together was essential. Treating the face and neck as a continuous unit eliminated any transition zone between the two areas and allowed for a smooth, uninterrupted contour from the cheek through the jawline and into the neck. The platysma muscle in the neck was also addressed to restore a clean, defined cervicomental angle.

Nano Fat Injection

Aging does not only cause sagging — it also causes volume loss. The fat pads in the face gradually deflate and descend over time, contributing to hollowing in the cheeks, under the eyes, and around the mouth. In this patient, nano fat injection was used to restore soft, natural volume to these areas.

Nano fat is harvested from the patient's own body, processed into a very fine consistency, and injected precisely where volume has been lost. Because it is the patient's own tissue, it integrates naturally with the surrounding structures and provides a soft, living result that synthetic fillers cannot replicate. Nano fat also contains a high concentration of stem cells and growth factors, which contribute to improved skin quality in the injected areas over time.

CO2 Laser Skin Resurfacing

The third component of this patient's treatment addressed the skin surface itself. CO2 fractional laser resurfacing was performed to improve skin texture, reduce fine lines, tighten pores, and stimulate new collagen production. This step complements the structural lifting and volume restoration by ensuring that the skin envelope itself looks refreshed and healthy — not just repositioned.

Results at 3 Months

The after photographs were taken at three months post-surgery. The jawline is visibly more defined, the neck contour is significantly improved, facial volume has been restored to a youthful balance, and the skin appears smoother and more vibrant.

The overall result demonstrates the advantage of a combined approach — rather than looking like one procedure was performed, the face appears naturally rejuvenated as a whole.
