Case Description

Hamide presented with excess abdominal skin, localized fat deposits, and stretch marks distributed across both the lower and upper abdomen. Her goal was a flatter, more contoured midsection. During the consultation, an important part of our discussion focused on setting realistic expectations — specifically regarding stretch marks and what surgery can and cannot achieve.

What Happens to Stretch Marks After a Tummy Tuck?

This is one of the most frequently misunderstood aspects of abdominoplasty, and Hamide's case provides a clear, honest example. A tummy tuck removes the skin between the belly button and the pubic area. Any stretch marks located on that lower abdominal skin are physically excised along with it — they are gone permanently. However, stretch marks that sit above the belly button remain. These marks are on skin that stays in place and is redraped downward during the procedure. They may shift to a slightly lower position on the abdomen after surgery, but they are not eliminated.

Hamide had stretch marks in both zones. The ones below her navel were removed entirely with the excised skin. The ones above her navel are still visible in her after photographs, and this is the expected and normal outcome. Being transparent about this distinction before surgery is essential, because patients who expect all stretch marks to disappear will be disappointed regardless of how well the surgery is performed. A well-informed patient, on the other hand, can appreciate the dramatic improvement in contour and skin quality while understanding the inherent limitations.

Surgical Approach

Hamide's lipo-abdominoplasty involved three key steps. First, liposuction was performed to reduce fat volume in the flanks and upper abdomen, creating a smoother transition between the treated area and the surrounding body. Next, the rectus abdominis muscles were plicated — sutured together along the midline to repair the separation that commonly develops after pregnancy. This step is what creates the flat, firm abdominal wall that exercise alone cannot achieve once diastasis recti is present. Finally, the excess lower abdominal skin was removed and the remaining skin was carefully redraped, with the incision placed low within the bikini line and the navel repositioned to sit naturally within the new contour.

Results at 7 Weeks

Seven weeks is an informative stage to document because the patient is well past the acute recovery phase but still in the active healing window. By this point, Hamide had returned to her normal daily routine and light physical activity. The compression garment phase was complete, and the tissues had begun to soften and settle into their final position.

The photographs at this stage show a significantly flatter abdominal profile, a more defined waistline, and a natural-looking navel position. Some residual swelling is still present — particularly in the lower abdomen, where fluid tends to accumulate due to gravity. This swelling continues to resolve gradually over the following three to five months. The scar is still in its early maturation phase, appearing pink and firm, and will continue to fade and flatten over the next 12 to 18 months.

The stretch marks above the belly button are visible in the after photographs, exactly as discussed during the preoperative consultation. Their presence alongside the clearly improved contour makes Hamide's case an honest and educational reference for prospective patients.

Surgeon's Note

I chose to highlight Hamide's case specifically because of the stretch mark discussion. In my experience, managing expectations is just as important as surgical technique when it comes to patient satisfaction. Patients who understand beforehand what a tummy tuck will and will not do are consistently more satisfied with their results than those who come in with unrealistic expectations — even when the surgical outcome is identical.

Technically, Hamide's result at seven weeks demonstrates good tissue healing and an effective muscle repair. The waistline definition from the liposuction is becoming apparent, and the abdominal wall is firm and flat. I expect her final result at six months to be excellent, with further improvement in scar maturation continuing well beyond that.

Frequently Asked Questions

Can a tummy tuck remove all stretch marks?

No. A tummy tuck removes only the stretch marks located on the skin between the belly button and the pubic area, because this is the skin that is physically excised during surgery. Stretch marks above the belly button remain on the skin that is kept and redraped. This is a physical limitation of the procedure and applies regardless of the surgical technique used.

How does the abdomen look at 7 weeks after a tummy tuck?

At seven weeks, the dramatic improvement in contour is clearly visible — the abdomen is flatter, the waistline is more defined, and the muscle repair has created a firm abdominal wall. However, some residual swelling is still present, especially in the lower abdomen. Patients have typically returned to their normal routine and light exercise by this stage. The final result continues to refine for another three to five months.

How long does it take for a tummy tuck scar to fade?

The scar goes through a maturation process that takes 12 to 18 months. In the first few months it appears pink, red, or slightly raised. Over time it gradually fades to a thin, pale line. The incision is placed low enough to be hidden by underwear and swimwear. Silicone-based scar treatments and consistent sun protection can help the scar heal as inconspicuously as possible.

Is diastasis recti repair included in a tummy tuck?

Yes. Repairing separated abdominal muscles — known as diastasis recti — is a standard part of a full abdominoplasty. The surgeon sutures the rectus muscles back together along the midline, restoring core support and creating a flat, firm abdominal profile. This muscle repair is often the most functionally significant part of the procedure and cannot be achieved through exercise once the separation has become significant.

What is the difference between results at 7 weeks and 6 months after tummy tuck?

At seven weeks the overall shape improvement is evident, but residual swelling — particularly in the lower abdomen — is still present and can fluctuate throughout the day. By six months, essentially all swelling has resolved, the tissues have fully softened, and the scar has flattened and lightened considerably. The contour appears more refined and the skin sits more naturally against the underlying structures.
