What Full Facial Rejuvenation Actually Means

The term "full facial rejuvenation" is used frequently in aesthetic surgery marketing, but its clinical meaning is specific: addressing every zone of the face that has aged in a single coordinated surgical session. For Angie, a 52-year-old patient who travelled from the United States to Istanbul, this meant four distinct procedures — facelift, neck lift, temporal browlift, and both upper and lower blepharoplasty — planned and executed as one unified operation. The rationale is straightforward. Facial ageing does not happen in isolation. The jawline softens, the neck develops laxity, the brows descend, and the eyelids accumulate excess skin and fat simultaneously. Treating only one zone risks creating a visible mismatch between the rejuvenated area and everything surrounding it.

Dr. Cem Berkay Sinaci, a fellow of the European Board of Plastic Reconstructive and Aesthetic Surgery (FEBOPRAS) and member of ISAPS and ASPS, approaches these comprehensive cases with a philosophy rooted in his advanced facial rejuvenation training — including fellowship under world-renowned plastic surgeon Raul Gonzalez in Brazil and cadaver dissection training in Bangkok, Thailand. The goal is never to create a "done" appearance but to restore what time has changed while preserving the patient's identity.

The Deep Plane Facelift and Neck Lift

The foundation of Angie's rejuvenation was a deep plane facelift with neck lift. Unlike older techniques that simply pull the skin tighter — creating the windswept, overtightened look that gives facelifts their outdated reputation — the deep plane technique works beneath the superficial musculoaponeurotic system, known as the SMAS. This is the layer of muscle and fascia that actually holds the facial structures in position. By releasing and repositioning the SMAS as a single composite flap along with the overlying fat and skin, the deep plane approach restores volume to where it has fallen rather than merely stretching the surface.

For Angie's neck, this meant addressing platysmal banding — the vertical cords that develop as the platysma muscle separates in the midline — and removing excess submental fat to recreate a clean cervicomental angle. The jawline was redefined not by pulling skin backward but by lifting the descended jowl tissue back to its original position above the mandibular border.

Temporal Browlift: The Upper Frame

The temporal browlift addressed the lateral brow descent that was contributing to heaviness in the outer portion of Angie's upper eyelids. Through incisions concealed within the temporal hairline, the outer brow was elevated to a position that restored the natural arch without creating a surprised or startled expression. This step was essential before performing the upper blepharoplasty — without it, too much eyelid skin might have been removed to compensate for what was actually brow-related heaviness, risking an unnatural result.

Upper and Lower Blepharoplasty

With the brow in its corrected position, the true extent of upper eyelid skin excess could be precisely measured. The upper blepharoplasty removed this redundancy through an incision within the natural crease, restoring a defined lid platform and eliminating the hooded appearance.

The lower blepharoplasty addressed herniated fat pads creating under-eye puffiness and the associated shadowing that contributed to a fatigued appearance. Through a subciliary incision placed just beneath the lash line, the fat compartments were accessed and the excess was addressed while preserving enough volume to avoid a hollow result. Skin redundancy in the lower lid was conservatively trimmed through the same approach.

For all eyelid incisions, Dr. Sinaci used non-dissolvable suture material. The eyelid skin is the thinnest on the body, and dissolvable sutures create an inflammatory reaction as they break down that can widen scars in this delicate tissue. Non-dissolvable stitches cause virtually no tissue reaction and are removed in four to six days, consistently producing finer, less visible scar lines.

Why American Patients Choose Istanbul for Facial Rejuvenation

Angie's decision to travel from the United States to Istanbul for her surgery reflects a growing trend among international patients seeking facial rejuvenation abroad. The considerations extend beyond cost. Turkey has become a global centre for plastic surgery, and Istanbul in particular offers a concentration of experienced, internationally trained surgeons performing high volumes of facial procedures. For American patients, the combination of board-certified expertise, modern surgical facilities certified by the Turkish Ministry of Health for international health tourism, and a recovery period that can be spent comfortably in one of the world's great cities makes Istanbul a compelling option.

What matters most, however, is the surgeon's specific experience with comprehensive facial rejuvenation. A combined facelift, neck lift, browlift, and blepharoplasty is not four separate operations performed sequentially — it is one integrated procedure where each component affects the others. The amount of skin removed during blepharoplasty depends on the browlift result. The facelift skin redraping depends on the degree of SMAS repositioning. The neck contour depends on the vector of the facelift. These interdependencies require a surgeon who routinely performs full facial rejuvenation and understands how each element interacts.

The Three-Month Result

Angie's before and after photographs at three months show a result that is nearing maturity. The jawline is sharply defined with the jowling corrected. The neck is clean and smooth with a restored cervicomental angle. The brows sit in a natural, youthful position without appearing elevated or artificial. The upper eyelids show a clean crease free of hooding, and the lower lids are smooth without the puffiness visible preoperatively.

At three months, the deep tissues have largely settled into their final position, though subtle refinement continues for up to twelve months, particularly in areas of deeper dissection like the neck and along the facelift flap. The scars — hidden within the hairline, around the ear, and within the eyelid creases — are maturing and will continue to fade. The overall impression is not of someone who has had surgery but of someone who looks rested, healthy, and like a refreshed version of themselves.

What Comprehensive Means for Long-Term Results

The advantage of addressing every ageing zone in a single session extends beyond the immediate result. Because all areas were rejuvenated simultaneously, they will age together going forward — there will be no mismatch between a youthful eye area and an ageing jawline, or a tight neck beneath descended brows. This synchronised ageing is one of the most underappreciated benefits of comprehensive facial rejuvenation and is a primary reason experienced facial surgeons recommend combining procedures when the anatomy warrants it and the patient's health allows it.
